Application fees are non-refundable. Please make sure you meet the eligibility requirements before you apply.
The application fee expires in one year. If your application expires before you have met all requirements, you will have to pay a re-application fee.
Change your address promptly. If you move during the application process, update your address immediately through your Online Licensing System account. Wrong addresses can delay your licensure.
Online verification. You may provide physical therapy services upon online verification of licensure prior to receiving your license certificate from the Board. This applies to both Permanent and Temporary licenses. You do not need to wait until you receive the license certificate from the Board. Verification of licensure is through the Online Licensing System, click on Public Search.
You can check the status of your application through your online account.
Your entry-level PT or PTA school code is required on the application. Go here for your SCHOOL CODE.
Applicants with Prior Criminal History. A review to determine licensure eligibility if you have a prior criminal history can be requested through your online account or by completing and submitting the Criminal History Evaluation Form available at ptot.texas.gov under the Request a Criminal History Evaluation link.
