This list contains all regular licenses issued by the Board. Results may include expired or voided licenses or temporary licenses for applicants who did not get a regular license. Please review your results carefully.

To print a Statement of Licensure or Wallet Card for a Current Active or Retired Status License, use the Last Name and DOB Search from the tab below.

“Search by Name” option:
A first or last name or a partial spelling may be entered. For example, you can search by last name “Smith” or first name “John” or a combination (for example, first name “J” and last name “Smith,” etc.).
 
“Last Name and DOB” option: 
You must search with both the last name and the date of birth (DOB). Dates must be entered in MM/DD/YYYY format. 
The full last name must be entered.
 

The agency is unable to assist with display or printing errors that may occur.
