Occupational Therapy License – Optional Items


See below information regarding accessing an optional statement of licensure or wallet card.

The agency is unable to assist with display or printing errors that may occur.

Note, in addition, the following, excerpted from §369.1 of the OT Rules, regarding the required display of the original wall license. The display of a statement of licensure or other item may not be used to satisfy this requirement. See the full OT Act and Rules for further regulations.

   (b) The original license must be prominently displayed in the licensee’s principal place of business as designated by the licensee. Reproduction of the original license is only authorized for institutional file purposes and not for public display.

If you are attempting to order a verification of licensure, see the Looking For Menu? to the left and select Send a Verification. Alternatively, licensees may submit the online application for such through their account in the Online Licensing System; see the User Guides from the same menu for further information regarding this option.

 

Statement of Licensure

  • Search for the licensee by using the Last Name and DOB search from the tab below.
  • You must search with both the last name and the date of birth (DOB).
  • Dates must be entered in MM/DD/YYYY format. 
  • The full last name must be entered.
 
 
Wallet Card
Only licensees with a current, unencumbered active status, retired status, or temporary license may access an optional wallet card. See the steps below to access such.
 
Steps
  • Log in to your account in the Online Licensing System.
    Only licensees who have already created an account in the system, which went into effect March 17, 2025, and have already linked their license to such may access the wallet card. If you have not already completed these prior steps, see the User Guides for related instructions.
  • Under Manage your license information, find your license.
  • From the drop-down menu on the right, select Print License Certificate and print such.
