13. What do I need to have for an audit?

For the audit, you will need to submit the required CE documentation. Refer to Chapter 367 of the OT Rules for the documentation requirements for CE categories, such as fieldwork supervision, in-service educational programs, Internet-based courses, etc. In addition, supporting documentation may include course descriptions, brochures, outlines, syllabi, and other course materials.  

12. Can I sign the renewal form affidavit before I finish the 24 hours?

No. Issues may arise that prevent you from completing the course. When you have finished all your CE, sign the form or go online to complete the renewal.
